At Gateway Dentistry Group, our aim is to provide you with the quality dental care that you deserve. We offer administrative support and patient information at our Grande Prairie dental office, including scheduling your first appointment. INFORMATION ABOUT INSURANCE AND PAYMENTS

Gateway Dentistry Group is committed to providing your family with professional dental care at affordable rates. We have payment options available for your convenience, and we accept all types of dental insurance. Our staff members are here to assist you with your insurance for all dental services, from direct billing for a checkup to paperwork for a crown. YOUR FIRST DENTAL VISIT

We cannot wait for you to visit us at Gateway Dentistry Group. We understand that you may be nervous before your initial visit, which is why we want to make things as smooth and simple as possible. When you come to your first appointment, you’ll need to provide the following information:

  • Any X-rays needed to assess your condition
  • List of medications you are currently taking
  • Dental insurance information (if you have insurance) that will allow us to assist you with getting any claims processed. This includes — your insurance company, policy number, the name of the subscriber plus the subscriber’s ID number and date of birth.
  • Complete medical history

Also, be sure to let our office know if you have a medical issue that could impact your dental treatment, such as rheumatic fever, diabetes, high blood pressure, a heart murmur or artificial heart valves or joints.

If a patient is under the age of 18, they will need to be accompanied by a parent or legal guardian at their initial appointment.

X-RAYS

X-rays are an important diagnostic tool we use to help us evaluate your overall oral health. The images captured by X-ray can help us identify and treat tooth decay, cavities, impacted teeth and other potential problems.

If you have X-rays of your mouth from a previous dentist or your physician, please request that they forward these images to our office. You can also pick up your X-rays and bring them with you to your appointment if you prefer. Our facility is equipped to take additional films if necessary.

SCHEDULING

Gateway Dentistry Group is proud to serve the community in and around Grande Prairie, Alberta. For your convenience, our hours are Monday through Friday, from 8 a.m. until 5 p.m. When you contact us, we strive to schedule all appointments as promptly as we can.

If you are experiencing intense pain or have an emergency situation, we will try to see you that same day.

FORMS AVAILABLE ONLINE

Before coming to Gateway Dentistry Group, you will need to download, print and fill out the following Patient Registration Forms. When you arrive for your first visit, bring them along. If you’re unable to print these at home, be sure to come 15 minutes early so you can fill them out before you see the dentist.
