Information about Insurance and Payments

Gateway Dentistry Group is committed to providing your family with professional dental care at affordable rates. We have payment options available for your convenience, and we accept all types of dental insurance. Our staff members are here to assist you with your insurance for all dental services, from direct billing for a checkup to paperwork for a crown. Your First Dental Visit

We look forward to meeting you at your initial visit. To make things as smooth as possible, we ask you to please assist us by providing the following information prior to or at the time of your appointment:

  • Any X-rays if applicable
  • A list of medications you are presently taking
  • If you have dental insurance, please bring the necessary information (insurance company, policy number, name of the subscriber, subscriber’s ID number and subscriber’s date of birth); this will save time and allow us to help you process any claims
  • Completed medical history and privacy policy if possible; you can print the form off from our website; if you cannot fill out ahead of time, we ask you to show up for your appointment 15 minutes early to fill one out

Please alert the office if you have a medical condition that may be of concern prior to dental treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, heart murmur, rheumatic fever, etc.) or if you are currently taking any medication (e.g., heart medications, aspirin, anticoagulant therapy, etc.).

Important: All patients under 18 years of age must be accompanied by a parent or guardian at the initial visit.

X-Rays

If you have X-rays from a previous dentist or physician, you may request they forward them to our office. If there is not enough time, please pick up your X-rays and bring them with you to your appointment. If additional films are necessary, they can be taken at our facility.

SCHEDULING

Gateway Dentistry Group is open Monday through Friday from 8:00 AM until 5:00 PM. We will schedule your appointment as promptly as possible. If you have pain or an emergency situation, every attempt will be made to see you that day.

Please download, print and fill out our Patient Registration Forms. After you have completed the forms, please make sure to bring it on your first visit to our office. The security and privacy of your personal data is one of our primary concerns, and we have taken every precaution to protect it.

Out-of-Town Surgery Patients

For those patients travelling from out of town, we would like to make your appointment easier. We suggest you print off the following items and bring them with you:

  • Your hotel name, address, telephone number and map
  • The address, phone number and map to our office
  • Privacy Policy
  • Pre-Op Physical
  • Before IV sedation and general anesthesia
